Piritramide ( R-3365 , trade names Dipidolor , Piridolan , Pirium and more ) is a synthetic opioid analgesic (narcotics painkiller) marketed in certain European countries including: Austria, Belgium, Czech Republic, Germany and the Netherlands. It comes in a free form, about 0.75x times as strong as morphine and administered parenterally (by injection) for the treatment of severe pain. Nausea, vomiting, respiratory depression and constipation are believed to be less common with pyriteramide compared to morphine (standard gold opioids compared to other opioids versus contrast), resulting in faster onset analgesia (pain relief) when compared with morphine and pethidine. After intravenous administration, the onset of analgesia is only 1-2 minutes, which may be associated with large lipophilicity. The analgesic and sedative effects of pyriteramide are believed to be potent with phenothiazines and their emetic effects (nausea/vomiting-induction) are suppressed. The volume of distribution is 0.7-1 L/kg after a single dose, 4.7-6 L/kg after steady-state concentrations are reached and up to 11.1 L/kg after dosing.

Piritramide was developed and patented in Belgium, in Janssen, in 1960. It is part of a class of two opioid members in clinical use with another is bezitramide (Burgodin). The closest chemical and structural relatives of pyriteramide in clinical use include the family of diphenoxylate, fentanyl (both Janssen's invention) and alfaprodin much further away.

Not currently in clinical use in the United States, it is a substance that controls Narcotics Schedule with DEA ​​ACSCN 9642 and zero manufacturing quotas.
